Our office will communicate with you beforehand a patient screening questionnaire to ask some screening questions for COVID-19 symptoms before making appointments. We recommend you take a printout of the forms, fill them, and bring them to the dentistry during your appointment to cut down the time you spent at the dental office. This will also allow us to serve your dental needs more quickly on your arrival.
